HIV Case Management
HIV case management is focused on meeting your immediate needs, while providing education and resources to strengthen the personal management of your health and care.
HIV case management will:
- Improve eligible client’s access to a wide range of appropriate services
- Promote continuity of care by coordinating service delivery arrangements
- Enhance client’s health status and level of functioning
- Promote efficiency by reducing or containing the overall cost of services
HIV case management includes assessment, care planning, resource development, service coordination, monitoring, reassessment and discharge.
