Effective Sept. 18, 2024, Vaya Health began providing statewide transition coordination for the following Money Follows the Person (MFP) participants:
- Older Adults: age 65 and older
- Individuals with physical disabilities transitioning via the Program of All-Inclusive Care for the Elderly (PACE) or the Community Alternative Program for Disabled Adults (CAP/DA) services
Key Highlights
- Transition services delivered by Vaya Health for older adults and individuals with physical disabilities under NC Medicaid Direct are funded with a grant from NC MFP.
- This does not affect existing contracts for MFP transitions using the Innovations or Traumatic Brain Injury (TBI) waiver programs.
Outreach and Education Efforts
Vaya Health will actively engage with:
- Potential MFP participants and their families
- Individuals and organizations involved in the transition process
- Stakeholders throughout the state
