Money Follows the Person Program Update

Vaya Health provides outreach, education and transition coordination.

Effective Sept. 18, 2024, Vaya Health began providing statewide transition coordination for the following Money Follows the Person (MFP) participants:

  • Older Adults: age 65 and older
  • Individuals with physical disabilities transitioning via the Program of All-Inclusive Care for the Elderly (PACE) or the Community Alternative Program for Disabled Adults (CAP/DA) services

Key Highlights

  • Transition services delivered by Vaya Health for older adults and individuals with physical disabilities under NC Medicaid Direct are funded with a grant from NC MFP.
  • This does not affect existing contracts for MFP transitions using the Innovations or Traumatic Brain Injury (TBI) waiver programs.

Outreach and Education Efforts

Vaya Health will actively engage with:

  • Potential MFP participants and their families
  • Individuals and organizations involved in the transition process
  • Stakeholders throughout the state
