Impacts NC Medicaid Managed Care Plans

This bulletin is a revision of the following: Tobacco-Related Policy Requirements Delayed until Jan. 1, 2027

Tobacco-related policy requirements will be effective starting on Jan. 1, 2027. This delay applies to:  

  • Standard Plans  
  • Behavioral Health and Intellectual/Developmental Disabilities (I/DD) Tailored Plans  
  • Children and Families Specialty Plan (CFSP)
  • Local Management Entity/Managed Care Organizations (LME/MCOs) contracted for behavioral health, I/DD and traumatic brain injury (TBI) service providers in Medicaid Direct

These requirements apply to both NC Medicaid and state-funded service providers.  

To support the health and well-being of NC Medicaid beneficiaries and state-funded service recipients, providers are encouraged to begin preparing now for the implementation of tobacco-free policies. While the deadline for implementation is Jan. 1, 2027, providers should:

  • Plan to begin preparation at least three to six months in advance to ensure a smooth transition, as this timeline may be needed to update existing tobacco use policies and treatment supports; and/or
  • Consider implementing these policies ahead of the deadline, which many providers have already done, to begin delivering health benefits earlier to the individuals they serve.

We encourage all providers to take proactive steps now to ensure readiness and support a successful transition.

Providers can access free training and technical assistance on the Breathe Easy NC website or join the statewide, monthly Change for Life Coalition to learn from other providers who have implemented tobacco-free policies successfully.
