Community Alternatives Program for Children (CAP/C)

The NC Medicaid Community Alternatives Program for Children (CAP/C) provides home and community-based services to children at risk for institutionalization in a nursing home.

Eligibility

CAP/C is available to any child from birth through 20 years of age who meets both the NC Medicaid  and CAP/C eligibility criteria which includes:

  • Lives in a private residence
  • Is able to be cared for safely at home
  • Requires the same level of care as a child in a nursing home or hospital
  • Has a family willing to participate in the care and in the care planning for their child

Services

Families are required to use one of the following additional services, at least once every 90 days:

  • In-home nurse or nurse aide care
  • Home modifications and vehicle modifications
  • Palliative care (art therapy, music therapy, counseling and bereavement counseling)
  • Caregiver training and education

Additional services include:

  • Respite care
  • Reusable diapers and disposable liners

Children on CAP/C have access to regular Medicaid services, including:

  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Medical equipment

Case Managers

All families on CAP/C will have a case manager to assist them with identifying their needs. The case manager will develop a plan of care to meet the child’s needs, monitor and coordinate the services and supplies in the plan of care.
