Use these free flyers and resources to explain Tailored Care Management to your community.

Tailored Care Management is an NC Medicaid service that pairs you with a specially trained health care worker who knows the health care system, a Tailored Care Manager. This service is available for Tailored Plan members and some people with NC Medicaid Direct.
