Tailored Care Management

Tailored Care Management (TCM) is a free service from NC Medicaid for people who have a serious mental illness, a severe substance use disorder, an intellectual/developmental disability or a traumatic brain injury.

Tailored Care Management pairs you with an expert who knows the system, a Tailored Care Manager.

Your Tailored Care Manager helps you navigate the health care system. They help manage your health and wellbeing — so you don’t have to do it all by yourself. They can even help with basic needs like food, transportation and housing.

For instance, your Tailored Care Manager can:

  • Find doctors, dentists and other specialists who take Medicaid and have openings
  • Schedule your appointments and follow up with your doctors
  • Arrange free rides to your appointments
  • Help you learn about managing ongoing health conditions
  • Help you get housing, healthy food and a job
  • Help you get a school Individualized Education Program (IEP)
  • Help you file government paperwork and advocate on your behalf
  • Be your main contact for all things Medicaid
  • And more!

Tailored Care Management is free, and it is your choice to use it. It does not change or replace your other services. You may have already been assigned a Tailored Care Manager who is ready to help you today.

You are eligible for Tailored Care Management if you have NC Medicaid with Alliance Health, Partners Health Management, Trillium Health Resources, or Vaya Health. Some exclusions apply.
