Max was homeless and was standing outside the homeless shelter. His Tailored Care Manager met him there and connected him to a temporary room in a rooming house.  

Max did not have a way to get to the rooming house, so his Tailored Care Manager arranged a free ride for him.  

He only had a trash bag of clothes. His Tailored Care Manager helped him by finding resources for furnishing his room. Max was able to get a mattress and other household essentials.

Max has had ups and downs since getting Tailored Care Management, but throughout it all he has stayed in contact with his team. They have built a good relationship and rapport together.  

Although Max already takes a lot of initiative in meeting his life goals, having a Tailored Care Manager ready to help him whenever he needs has been so helpful.  

Together, Max and his Tailored Care Manager are working to find permanent housing and a stable job.

Sam was completely blind in one eye.

Every year, Medicaid has covered the replacement of his specially made contact. Unfortunately, Sam lost his contact, but it wasn’t time for his replacement yet.

Her Tailored Care Manager helped Sam get a replacement lens at no cost!

John had recently been released from prison. He was supposed to start a new job, but he didn’t have a car. He also really needed to see a dentist.  

His Tailored Care Manager was there for every step of his transition. She arranged transportation for him as John got things in order for his new job. She scheduled a much-needed appointment with a dentist that takes Medicaid.  

Now John feels excited about his future!

Eric was in and out of the emergency room weekly, sometimes daily. At one of these incidents, the hospital staff got in contact with his Tailored Care Manager.

His Tailored Care Manager worked closely with the hospital social worker. They found a close family member that could help support Eric.

Eric agreed to work with his Tailored Care Manager. He was able to get safe housing and daily meals with the help of his Tailored Care Manager.

Before he met his Tailored Care Manager, Eric said it had been many years since he had health care.

Now, his Tailored Care Manager is helping Eric find a primary care doctor.

Kate had a lot of health issues with her stomach. She began to lose a lot of weight. When she met her Tailored Care Manager, Kate shared that she has had stomach issues for nearly a year.  

Because of her mental health, doctors refused to do an endoscopy and treatments she needed. She was upset and did not know what to do. At a certain point, Kate began to lose her vision.  

Her Tailored Care Manager found a doctor who was willing to work with Kate. She helped set up a free ride for the appointment.  

The doctor found out Kate had a virus that was treatable with antibiotics, and Kate’s health improved.

Don’t see your plan? Tailored Care Management is only available with Tailored Plans or NC Medicaid Direct; it is not available with Standard Plans or Eastern Band of Cherokee Indians (EBCI) Tribal Option. See contact information for all Medicaid health plans.

Not sure who to call? Call the number on your health plan ID card. Or call the NC Medicaid Enrollment Broker at 1-833-870-5500. Hours of operation: 7 a.m. to 5 p.m., Monday through Saturday. They can tell you which plan you have.

If you have a complaint or concern, or you need help understanding your rights and responsibilities: Call the NC Medicaid Ombudsman at 1-877-201-3750. Hours of operation: 7 a.m. to 5 p.m., Monday through Saturday. They are an outside group that can advocate on your behalf.
