New LTSS Care Management Webpage
NC Medicaid has launched the Long-Term Services and Supports (LTSS) Care Management program webpage. This page provides valuable information on the LTSS Care Management program and comprehensive information for providers and beneficiaries, as well as houses the LTSS Care Management Program Guide. The program guide is intended to guide health plan development of Care Management practices for members with LTSS needs. Advanced Medical Home Provider Manual 2.1 Released
NC Medicaid has released an update to the Advanced Medical Home (AMH) Provider Manual that provides clarification to the Well-Child Measures, according to the measure steward and listed under Quality Metrics, as well as addresses several grammatical issues. This manual replaces the previous version published in February 2021. The manual can be found under the Resources section of the Advanced Medical Home webpage.
Program Guide Added
A program guide for Care Management of High-Risk Pregnancies and At-Risk Children in Managed Care has been added to the corresponding web pages. The program guide is intended to be a reference guide for health plans in Care Management for High-Risk Pregnancies and At-Risk Children.
