Medicaid Enrollment of Healthy Opportunities Human Service Organizations Human Service Organizations are required to enroll as Medicaid providers through NCTracks.

Human Service Organizations (HSOs) contracted with the Network Leads selected by NC Medicaid to oversee the delivery of the Healthy Opportunities Pilot services, are required to enroll as Medicaid providers through NCTracks. Providers can access:

  • The HSO Provider Enrollment Participant User Guide in the SkillPort library on NCTracks:. 
  • Providers can also access User Guides and Fact Sheets.

The provider enrollment application process requires HSOs to first obtain a North Carolina Identity Management number (NCID), as the NCID serves as the user ID that provider access to the portal (ncid.nc.gov). Upon entering the user ID and password, Multi-factor Authentication (MFA) is required. For information on the MFA process, providers should refer to the “Provider Multi Factor Authentication Registration Process” Job Aid also located in SkillPort.

As part of the enrollment process, NCTracks requires taxonomy codes to be entered on the Taxonomy Classification screen of the online application. It is important that HSOs at least select the following taxonomies:  

  • For Individuals: Taxonomy code 405300000X, which is a new Level II code for Prevention Professional 
  • For Organizations: Taxonomy code 251K00000X, which is a new Level II code for Public Health or Welfare

HSOs selecting only these taxonomies are exempt from paying the federal enrollment fee and from the reverification process. If another taxonomy is selected, the provider must meet the criteria as outlined in the Provider Permission Matrix including federal enrollment fee, fingerprinting and site visit requirements as applicable to the selected taxonomy.  

In addition, all newly enrolling providers must complete provider training as legislatively mandated in NCGS Chapter 108C-9. Notification of the required training is received from Public Consulting Group (PCG). Any provider that receives notification of training must complete it within the time period specified to avoid denial or termination.
