Primary Care Provider/Advanced Medical Home Auto-Assignment Fact Sheet Now Available

The fact sheet outlines the key responsibilities of practices receiving auto-assigned members.

NC Medicaid has released a new fact sheet to support provider understanding of the Primary Care Provider (PCP)/Advanced Medical Home (AMH) auto-assignment process under NC Medicaid Managed Care.

The PCP/AMH Auto-Assignment Fact Sheet:

  • Clarifies how members are assigned to PCPs and AMHs
  • Outlines key responsibilities of practices that receive auto-assigned members.
  • Includes resources and tools such as:
    • Standard change request forms from Prepaid Health Plans (PHPs)
    • NCTracks guidance
    • Panel management materials

The Fact Sheet applies to all practices participating as PCPs/AMHs under NC Medicaid Managed Care Standard Plans.

Reminder: AMH/PCPs may still serve and bill for NC Medicaid beneficiaries who are not assigned to them.

NC Medicaid is reviewing and monitoring the application of assignment algorithms and considering potential updates to assignment rules to improve the accuracy and stability of PCP/AMH auto-assignments. Any updates to PCP/AMH auto-assignment rules will be communicated as they are finalized.
