Update on Request to Move for Children and Families Specialty Plan Members

Request to Move forms for members in the Children and Families Specialty Plan to move to a Behavioral Health I/DD Tailored Plan do not need to complete a Service-associated Request.

All Request to Move (RTM) forms submitted to the Enrollment Broker for members in the Children and Families Specialty Plan (CFSP) will be processed as Non-service Associated Request beginning Dec. 1, 2025. All plan changes for the CFSP members will take effect the first of the following month.

All CFSP members can choose to disenroll from the CFSP and enroll in a Standard Plan. Some members can choose a Tailored Plan based on their Tailored Plan eligibility determination. Call the Enrollment Broker to learn about the members’ choices. If a member is not eligible, use an RTM form.

Switching out of the CFSP to another plan may result in loss of statewide access to specific services, depending on the location of the member and services available.

To submit a Request to Move form, visit the NC Medicaid Enrollment Broker’s website and use the guided forms page. Forms may be submitted via mail or fax. Contact the Enrollment Broker at 833-870-5500, 7 a.m. to 7 p.m., Monday through Friday.
