Application Period Open for Round 3 Tailored Care Management Certification

A key area of focus in Medicaid Transformation is building robust and effective models for supporting beneficiaries’ whole person care needs through local care management. Provider-based care management will support whole-person care, foster high-functioning integrated care teams, address medical and non-medical drivers of health and promote better health outcomes.  

The Round 3 process for Tailored Care Management (TCM) Certification is now open. 

  • To assist providers in understanding the process and requirements, the Department will host a webinar Thursday, Sept. 29, 2022, at 3:30 p.m. to provide more information and respond to providers’ questions. 
  • Please register on the Mountain Area Health Education Center (MAHEC) website.

The certification process will be conducted by the State’s certification vendor, the National Committee for Quality Assurance (NCQA), using the State’s Tailored Care Management model requirements. 

  1. To participate, providers must complete the TCM Statement of Intent to Participate to receive a log-on to the application portal. The TCM Statement of Intent is an online form and will be available until Nov. 10, 2022
  2. The Round 3 application will be available online from Nov. 15, 2022, until Dec. 31, 2022. The deadline for submitting applications and associated documents is Dec. 31, 2022.

Interested providers should review the updated Tailored Care Management Provider Manual, which describes the model, TCM requirements and gives guidance on the certification process for AMHs and CMAs.
