Effective Oct. 1, 2026, the Beneficiary Information section of the NC 1915(i) Assessment Tool will be updated to include the name of the newly formed health plan Vaya Partners.
This name will replace Vaya Health and Partners Health Management, which are currently listed in the assessment tool. Please ensure tailored care managers and care coordinators are using the updated version of the assessment tool.
