NC Innovations Supports Intensity Scale

The Supports Intensity Scale® (SIS) was developed by the American Association of Intellectual and Developmental Disabilities (AAIDD).

SIS assessments identify what help a person with intellectual or developmental disabilities needs to do the same things that others without disabilities do every day. It measures the assistance needed to shop, prepare meals, get around from place to place, work, have friendships, and other daily life activities.

The SIS is being used for two purposes:

  • To guide development of Person-Centered Service Plans, and
  • To set Individualized Budget Amounts to be used for services.

SIS Assessment

  • A trained SIS interviewer leads a discussion of your need for support in different life activities.
  • The SIS interviewer is well-trained and should help you feel comfortable.
  • The SIS interviewer will use the same list of questions for you and everyone else. These questions are about the amount of assistance, if any, you need in each daily life activity area. The SIS interviewer documents how much support you need.
  • You should attend the SIS meeting and participate with other people who know you very well. A SIS meeting usually takes between two to three hours.
  • There are no right or wrong answers to the SIS questions. Explain what your needs are and let people know your abilities when you can. If you do not understand a question, ask the SIS interviewer to explain it. It is important that all answers are accurate and not over or underestimated.
