Justice Involved Youth Care Management

Helping youth successfully transition from placement to the community

If you are a Medicaid-enrolled child or youth involved with the juvenile justice system, you may be eligible for Care Management services designed to support your health, well-being, and successful transition back into the community. As part of federal requirements under Section 5121 of the Consolidated Appropriations Act (CAA), eligible youth can receive personalized care coordination before release and continued support after returning home.

You may be eligible for Care Management services if*:

• You are enrolled in NC Medicaid
• Are under age 21
• Are currently placed in a post-adjudication juvenile justice facility, such as a Youth Development Center (YDC) 
• Are preparing for release from a juvenile justice placement within the next 30 days
• Have recently returned to the community following release from a juvenile justice placement within the last 30 days (Effective Dec. 20, 2026)

If you are eligible for Care Management here is what’s included in your benefits:

Screening and Diagnostic Services

• Physical examinations
• Vision, hearing and lab testing
• Dental screening services
• Immunizations
• Comprehensive health needs assessments (e.g., behavioral health, health-related social needs)

Individualized Care Planning

Together with your care manager, you will develop a person-centered care plan tailored to your unique needs and goals.

Your care manager will help:

• Develop a personalized care plan based on your health, behavioral health and support needs
• Coordinate services across health care providers, schools, community organizations and juvenile justice partners
• Manage referrals and help schedule appointments and services
• Connect you with community resources and support that aligns with your goals
• Monitor your progress and update your care plan as your needs change
• Support smooth transitions during and after release to ensure continuity of care

Additionally, you will receive full Medicaid coverage for up to 12 months after release via NC Medicaid Direct. After this period, you will transition to the Medicaid delivery system that best meets your ongoing needs and circumstances. Please note that Tailored Care Management is not available with Standard Plans or Eastern Band of Cherokee Indians (EBCI) Tribal Option.
