How to Get 1915(i) Services
First, please check the eligibility requirements for 1915(i).
1. Call and ask for a 1915(i) assessment
These NC Medicaid health plans offer 1915(i) services. Call the number listed on your health plan ID card (or health plan welcome letter) and ask for a 1915(i) assessment:

- Vaya Health, 1-800-962-9003, TTY: 711
Don’t see your plan? 1915(i) services are only available with Tailored Plans, NC Medicaid Direct or Eastern Band of Cherokee Indians Tribal Option. 1915(i) services are not available with Standard Plans. See contact information for all Medicaid health plans.
Not sure which plan you have? Call the NC Medicaid Enrollment Broker at 1-833-870-5500. Hours of operation: 7 a.m. to 7 p.m., Monday through Friday.
If you have someone who is assigned to help you with your Medicaid (like a Tailored Care Manager or Care Coordinator), you can call them instead.
2. Complete the assessment
You will be contacted to schedule a 1915(i) assessment. The purpose of this assessment is to see which 1915(i) services you need.
The assessment can be done in person or through a telemedicine appointment. This can take place at your home or in a convenient place. It can take up to an hour.
You should get services within 90 days. There may be other factors, like staff availability, that determine when services can start. The person who helped you with your assessment can give you updates (this could be your Tailored Care Manager). Or call the Member and Recipient Service Line listed on your health plan ID card.
