Care Coordination for Children (CC4C)

Care Coordination for Children (CC4C) offers community-based interventions for children. This program targets the highest risk and highest cost children for care management. CC4C services are determined by the beneficiary’s needs and plan of care.

Contacts may occur in various settings, including:

• Community or beneficiary’s home
• Beneficiary’s phone
