What is Money Follows the Person?

Money Follows the Person (MFP) is a federally funded NC Medicaid program that helps people living in hospitals, nursing homes, or care centers for people with disabilities move into homes in their communities with the support they need.

The MFP program:

  • Helps people move from facilities (like hospitals, nursing homes or care centers for people with disabilities) into homes or apartments
  • Gives more choices in where and how people get help
  • Looks for ways to fix problems that make it hard for people to get care at home

Who’s Eligible?

To qualify, a person must:

  • Live in a hospital, nursing home or care facility for people with disabilities for at least 60 days
  • Be eligible for community-based Medicaid
  • Be approved for home and community-based services
  • Choose to move into a ”qualified home”, such as:
    • Their own home or apartment
    • A family member’s home
    • A group home with four (4) or fewer people, in limited cases

Faster Access to Services

  • Special Medicaid programs and supports

Help with Moving

  • Rent and utility deposits
  • Furniture and home modifications (e.g., ramps)
  • Staff training
  • One-time needs like food

Ongoing Support

Participants receive services through one of these NC Medicaid programs:

  • CAP/DA (Community Alternatives Program for Disabled Adults)
  • Traumatic Brain Injury (TBI) Waiver
  • NC Innovations
  • PACE (Program of All-Inclusive Care for the Elderly)
