Hospice services provide medical care, support and comfort to people who are terminally ill and near the end of life. They also offer help to the person’s family and caregivers.
When someone chooses hospice, it may limit other types of services NC Medicaid will pay for. Hospice benefits cover all care related to the person’s end of life illnesses.
Hospice Evaluation
An interdisciplinary team, made up of doctors, nurses, social workers and other supportive staff, creates a care plan. This plan explains how the team will meet the person’s medical, emotional and spiritual needs. Hospice care can be provided in a private home, a hospice center, an adult care home, a nursing home or a hospital if there is an agreement with the hospice agency.
